There are 2 steps to book an assessment with us.
2. Please send completed forms via email to firstname.lastname@example.org or via postal services to 68 London Street, Richmond Christchurch 8013.
Any information you can tell us is helpful. If you need help to fill in this form, please complete as much as possible then ring our receptionist. Our goal is to phone you, to confirm we have received the application, within 3-5 working days of receiving this information.
We will call you again, as soon as an appointment becomes available. As of July 2021, the waiting list was 6 months.
Once we have received the completed form, our receptionist will email or ring you between 9am - 4pm Monday to Friday (estimated within 3-5 working days) to confirm receipt of the form and to discuss the appointment.
A confirmation letter of appointment details will be sent to you (either via email or post)
